If you are injured on the job, please fill out a Occupational Injury Report, have your supervisor sign and give to HR ASAP
Also if you feel you need to be seen by a doctor, Monday – Friday you may call the HR office and they will authorize the visit for you, if you cannot reach HR , you may contact the main front desk and they will be able to help you.
If it is on the weekend, you may go directly to Concentra Urgent Care with out authorization and of course if it is a life threaten injury go to the EMMC Emergency room.
Make sure Human Resource receives all paper work.
