Early Intervention Case Management 
Early Intervention Case Management (a MaineCare-only service) is available to families who have a child (under age 6) with a developmental delay or at risk of a delay. No medical diagnosis is necessary. One of our caring professionals will meet with you to assess your child’s needs. This service can be self-referred by you, the parent. Medical professionals can also make referrals directly to UCP of Maine.
Targeted Children’s Case Management
Our Case Managers link children and families to various community programs and resources, and help them access educational, medical, and psychological services so they can meet their needs and achieve their goals. Targeted Case Management is available to children with developmental disabilities, a mental or behavioral health diagnosis, or a problematic medical diagnosis.
Community Case Management (Adult Case Management)
A service for adults who are eligible for developmental services and who are diagnosed with developmental disabilities and/or Autism Spectrum Disorders. CCM works to assist adults with gaining access to education, employment, housing, and healthcare, as well as many other resources that the community has to offer. Each adult will participate in developing a Person Center Plan (PCP) with their team of providers that helps to address their needs and desires.
