What is Shared Living?

Shared Living is a 24/7 home support service for people with Section 21 or Section 29.

Examples of Shared Living:

  • Living with your family
  • Living with an unrelated person you know
  • Living with an unrelated person and/or their family

Why should I consider Shared Living?

  • It offers a safe home support option
  • Access to 24/7 support in home and to access the community
  • A consistent place to call home
  • Support from the same person; so they know and care about you
  • Support based on your skills, abilities, desires, and needs
  • Privacy and freedom
  • Personalized support to work towards living the life you want

Potential Shared Living Clients

  • If you know who you want to be your provider, you and your team can contact Kristina to start the process.
  • If you do not have a provider in mind, give us a call and we can see if we have any available providers to match you with.

Potential Providers must apply and be approved as a contractor. UCP providers the required Direct Support Professional (DSP), CRMA, and CPR trainings.

Once members and providers are matched and trainings are complete, your team will meet to update your Person Centered Plan (PCP) and select a start date for service.
